If your student requires medication during the school day or needs to have medication at school for asthma, seizures, allergies, anxiety, or any other reason, please don't forget to have the appropriate form completed and turned in by the first day of school. Medication can not be administered without a complete form that includes a signature from the prescribing doctor and the parent/guardian. All prescription medication must be in a currently dated bottle or package. Medications and completed forms may be turned in on orientation day. These forms are available on our website here.
